The premise
A homeless man meets a medical school student who pays him to volunteer for a surgical procedure known as trepanation, drilling a hole in his skull, which ends up giving him the ability to communicate with the dark side of people’s subconscious minds.

A destitute man living on the margins of society agrees to undergo a radical, unauthorized surgical experiment — a hole drilled into his skull — in exchange for money. The procedure leaves him with a disturbing new faculty: the ability to perceive the buried psychological wounds of those around him in visceral, hallucinatory form. Adapted from a celebrated manga, this Japanese film blurs the line between gift and curse as its protagonist is drawn deeper into the fractured inner worlds of strangers. What begins as a tale of desperation evolves into a haunting meditation on the hidden darkness we all carry.
