The premise
Pauline, a disturbed, outcast teenager, has a fascination with surgery and experiences violent, psychosexual fantasies, all while aspiring to a career in medicine.

Pauline is a socially awkward, fiercely intelligent teenager whose dreams of becoming a surgeon coexist with increasingly disturbing fantasies involving blood and bodies. At home she clashes with her overbearing mother, while at school she exists firmly on the outside looking in. Director Richard Bates Jr. wraps this portrait of a deeply troubled mind in pitch-black comedy, making it hard to look away even when you desperately want to. It's a portrait of adolescence taken to its most unsettling extreme.
