The premise
An ophthalmologist's assistant with an unhealthy interest in human eyeballs goes on a killing spree to collect eyeballs for his overbearing mother's collection. Reality soon takes a bizarre turn, both for the characters and the audience.

A meek ophthalmologist's assistant, dominated by his possessive mother, embarks on a gruesome hunt driven by a deeply disturbing obsession with human eyes. Spanish director Bigas Luna constructs a film that aggressively toys with the boundary between viewer and viewed, pulling the audience into a disorienting meta-layered nightmare. Equal parts grindhouse shocker and psychological puzzle, Anguish is a rare horror entry that weaponizes the very act of watching a movie against you.
