The fourth entry in Japan's notorious Guinea Pig series takes a sharp comedic turn, following a deranged doctor whose experimental procedures on unsuspecting patients produce spectacularly grotesque results. Equal parts black comedy and splatter showcase, it leans into camp and outrageous practical effects rather than straight-faced dread. Short in runtime but relentless in its assault on good taste, it occupies a unique niche even within its infamously extreme franchise.
