The premise
When a single mother suffers a nervous breakdown, she is suspected of child abuse and her child is taken away. Her mental suffering escalates as she succumbs to her darkest fantasies.
Horror Vault's take
Kotoko plunges into the fractured psyche of a single mother whose grip on reality begins to unravel, blurring the line between what is real and what her tortured mind conjures. Directed by Shinya Tsukamoto, the film is an unflinching portrait of psychological disintegration set against the overwhelming pressures of motherhood. Raw, intimate, and deeply unsettling, it channels its horror not through monsters but through the terrifying landscape of a mind in crisis.
